Immunization Requirements
All new students are required to complete the Immunization Form (see dates below). No physical exam is required to attend North Central College. If the immunization information is not complete, a medical hold will be placed on your account two weeks after the first day of classes of your starting term. This means that you cannot add a class or register for the next term.
Illinois State Law mandates that all students entering a post-secondary institution, born after 1956, must provide proof of immunity to measles, mumps, rubella, diphtheria and tetanus.
Deadlines for immunization requirements:
If you start:
Fall Term Deadline is August 1st
Winter Term Deadline is December 15th
Spring Term Deadline is March 15th
All transfer students are required to show proof of immunizations within first two weeks of class for the term they start at North Central College.
Where can I locate my immunization information?
You may be revaccinated with 2 doses of MMR vaccine, and one dose of Tdap vaccine doses must be 28 days apart or you may have a lab test done to show immunity.
Where can I get immunizations?
You can get immunizations at your primary health care provider office, the county health department where you live, and walk-in clinics. We provide MMR And TDAP vaccines at Dyson Wellness Center for a fee which can be put on your student account.
The Dyson Wellness Center also offers the TB test for a fee.
All international students will have to have a TB test done upon arrival.

- Military records
- Primary Health Care Provider
- Childhood records
- 2 doses of measles, 1 dose of mumps and rubella - after 1 year of age.
- Tetanus/Diphtheria (Td) or Tetanus/Diphtheria/Pertussis (Tdap) within 10 years
- New International students: Tuberculin skin test (PPD)
- Meningitis
- Varicella (chicken pox)
- Hepatitis B
- Gardasil
